The following information will be used for the Affidavit of Assessment Work (Proof of Labor), Notice of Intent to Hold and/or BLM Maintenance fees. Please click the button below to fill out the form – it will be used to submit annual assessment work by the Claim Chairperson.
This form should be submitted by members or their guests who have worked on any VP club claim. Please submit one form for each claim you have worked on. Your comments and concerns will be read by the Claims Chairperson.
